Dementia Support

Compassionate Care for Every Step of Your Journey

At TRU Community Care, we understand the challenges that come with supporting a loved one living with dementia. Our dementia support program is part of a national Medicare demonstration project called GUIDE—Guiding an Improved Dementia Experience.

Through GUIDE, we provide in-home, coordinated care to help people live safely and meaningfully in the place they call home.

We’re here to walk beside you—offering expert guidance, emotional support, and practical resources.

What You Can Expect

Our services are designed to:

  • Help reduce emergency visits and hospitalizations
  • Support you in managing behavioral symptoms of dementia
  • Provide tools, training, and relief for care partner stress

You’re not alone—we’re here to help lighten the load.
