At TRU Community Care, we understand the challenges that come with supporting a loved one living with dementia. Our dementia support program is part of a national Medicare demonstration project called GUIDE—Guiding an Improved Dementia Experience.
Through GUIDE, we provide in-home, coordinated care to help people live safely and meaningfully in the place they call home.

📝 Personalized Assessment
We collaborate with your loved one’s healthcare providers to create a plan tailored to their needs.
🤝 Ongoing Support at Home
Dedicated TRU care team with 24/7 phone support.
📚 Care Partner Education and Support
We will provide care partners with training, tools, education, and coordination with local services and supports.
⏳ Respite Care*
Short-term relief to support your wellbeing as a care partner. *Applies to individuals who qualify through CMS for this type of support.
Who Qualifies?
An individual may be eligible for this program if they:
- Have been diagnosed with dementia
- Are enrolled in Medicare Parts A and B
- Are NOT enrolled in Medicare Advantage Plan
- Live in an independent residence or assisted living facility
There is no additional cost—services are covered under your Medicare benefits.

Get Started in Four Easy Steps

TRU’s Dementia Support Program begins in July 2025. If you or someone you love is living with dementia, we’re here to help you navigate the next steps with confidence.
- Call TRU Community Care at (303) 449-7740 or fill out the online interest form.
- We’ll contact you to schedule an initial discovery call.
- If eligible, you will receive documentation to complete. Our team will guide you through the documentation submission, approval status, and next steps.
- Once approved by Medicare (CMS), TRU will schedule and in home assessment, and you will have access to your TRU dementia care team and services.
