TRU Community Care and The Conversation Project Join Forces

Two local organizations providing important end-of-life resources are coming together to better serve the Boulder County community. Acknowledging that end-of-life planning should be done far in advance of developing a serious or terminal illness, TRU Community Care (TRU) recognized that The Conversation Project in Boulder County (TCPB) approach and materials would be a welcome addition to their community education efforts.

TRU, founded in 1976 as Boulder Hospice, has been providing hospice care, palliative care, and grief services for 43 years. In 2017, TRU PACE (Program of All-Inclusive Care for the Elderly) began in order to offer comprehensive care services designed to help people continue to live as independently as possible. The Conversation Project in Boulder County formed in 2013, based on the nationally-acclaimed work of Boston Globe columnist and author, Ellen Goodman, with a mission to foster meaningful and effective conversations around care at the end-of-life, resulting in the appointment of an informed decision-maker.

Over half of us will be unable to speak for ourselves at the end of life. Choosing and documenting your choice of a decision-maker is an important first step. Talking to that person and your family is equally important. Starting the conversation is never easy, but families and health care professionals report that it is a relief when the subject of how we want the end of our life to look is brought into the open and our choices can be honored because of careful forethought and conversation.

TRU Community Care is excited to build on the work of The Conversation Project and to help community members, palliative care and hospice patients, and PACE participants begin to have these critical conversations earlier in their journey. The Conversation Project will retain its name, volunteers, and identity and looks forward to being part of TRU’s community offerings.

TRU Community Care Awarded $250,000 Grant from Weinberg Foundation

The Harry and Jeanette Weinberg Foundation logo(Lafayette, CO) — TRU Community Care is the recipient of a $250,000 grant for its capital campaign. Awarded by The Harry and Jeanette Weinberg Foundation of Baltimore, Maryland, the grant brings the total giving to almost $5 million, just shy of TRU’s $6 million goal. With this grant, the Weinberg Foundation will name the rehabilitation gym in the TRU PACE Center, where participants receive physical, occupational, speech and other necessary therapies.The capital campaign began in 2015 to raise funds for the new TRU PACE and Grief Services building located in Lafayette. PACE stands for Program of All-inclusive Care for the Elderly.

TRU Grief Services supports hospice families and the community at large with individual grief counseling and support groups. About 2,500 individuals each year benefit from services. Thirty percent have been served by TRU Hospice, and 70% are community members who have suffered some type of loss.

TRU PACE is now enrolling eligible seniors who live in Boulder and southwest Weld Counties. This innovative program, which began on March 1, serves residents by providing preventive, primary, acute and long-term care services that enable these elderly individuals to continue living safely in the community.

The Weinberg Foundation is committed to helping low-income older adults age in their communities with independence, dignity, and maximum quality of life,” said Aaron Merki, Weinberg Foundation Program Director, Older Adults. “PACE programs, including TRU PACE, play a vital role in fulfilling that commitment by providing and coordinating a variety of supports and services.

TRU PACE is available to people 55 years or older who are certified by the State of Colorado to need nursing-home-level care. The program supports clients to live as independently as possible and helps participants and their families navigate the complicated issues associated with aging. Individuals in Boulder County or southwest Weld County, who are currently living safely in the community, are encouraged to call and inquire about enrolling in TRU PACE.

TRU PACE’s goal is to help seniors in our community age in place,” says Executive Director Bob Thorn. “With the aging population in our community projected to grow 69% during the next 25 years, TRU Community Care is seizing the opportunity to continue its leadership by adding a Program of All-inclusive Care for the Elderly (PACE) to our cadre of services.

As a program that serves seniors from all walks of life, TRU PACE is a Medicare/Medicaid covered program for participants. Clients in the TRU PACE program must receive all their health care, primary care and specialty physician services (other than emergency services) from TRU PACE providers. TRU PACE is part of the National PACE Organization (www.npaonline.org).

Michael McHale from Arlington Heights, Illinois, most recently President and CEO of Rainbow Hospice and Palliative Care, will begin his leadership of TRU Community Care, TRU Hospice of Northern Colorado, and TRU PACE (Program of All-inclusive Care for the Elderly) on September 11, 2017.

McHale has dedicated his professional career to health care with more than 17 years of management, marketing and customer service experience within the hospice and long-term care industry. He has held various leadership positions with hospices in California, Michigan and Maryland prior to joining Rainbow Hospice and Palliative Care in Chicago. Michael holds a Masters of Health Care Administration from National University in San Diego. As a board member of the National Hospice and Palliative Care Organization (NHPCO) in Washington, DC, he is committed to improving quality of care, regulatory oversight and patient and family satisfaction in hospice, palliative medicine and PACE.

This new chief steps in at a critical time in the organization’s history. With over 40 years’ experience in the delivery of end-of-life care, TRU is continuing to grow its programs, having launched TRU PACE in March 2017.

TRU has an outstanding reputation of being a leader in caring for those living with advanced illness. It is an honor for me to join this organization to continue to build on this reputation and find new ways to care for our community.

TRU Colors Art Sale to Benefit TRU Hospice

(Boulder, CO) – TRU Hospice Thrift Shop, located at 5565 Arapahoe Avenue in Boulder, is collaborating with Mary Williams Fine Arts at 5311 Western Avenue in Boulder to offer “TRU Colors Art Sale” from June 26-30, 2017. Opening night is Friday, June 23rd and will include a reception with refreshments, best selection on limited-edition prints and original works of art, and door prizes.

Using donations of wall art, sculptures, and blown glass from the thrift shop and community at large, art enthusiasts can add to their collection. The professionally vetted art will be sold with 100% of the proceeds benefiting TRU Hospice.

A limited-edition print by popular culture icon and artist Peter Max will be one of the featured pieces in the sale. His bold colors, uplifting images and an uncommon artistic diversity have touched almost every phase of American culture and inspired many generations. Max’s signature style of cosmic characters meticulously painted against bold, vibrant colors was among the most influential graphic sources of the 1960s.

Volunteers at the TRU Hospice Thrift Shop work to identify items for eBay that attract a higher price than items in the shop. Dan Gleeson, former art teacher at Boulder High School observes, “We are amazed at the quality of the items donated to benefit the work of TRU Hospice. I’m so excited to pull together a collection for sale at Mary Williams Fine Arts—it will be a great opportunity for us.”

TRU Hospice Thrift Shop opened in 2005 to offer a depository for estate items to help families who have lost a loved one. When families feel burdened by possessions, it’s a gift to have TRU Hospice help find a new home for the furniture and household goods. Collectibles, books, furniture, art, and sporting goods, among other contributions, are accepted daily at the shop in east Boulder.

TRU Hospice Thrift Shop seeks volunteers for 4-hour shifts each week. More than 100 volunteers offer their services to the shop and find a great sense of community in the work. Paige Foundation Steps Forward to Support Seniors and New TRU PACE Center

(Lafayette, CO) – The Herbert & Judy Paige Family Foundation is impacting senior care in Boulder County and beyond through funding and lending to TRU Community Care. Through a shared Board Member, Timothy Schafer, a local attorney specializing in estate planning and a native of Broomfield, TRU has benefited from the Paige Foundation through a low-cost loan of $1.5 million to help with program start-up costs associated with TRU PACE and a capital grant of $40,000 to help offset construction costs.

TRU PACE (Program of All-inclusive Care for the Elderly) launched on March 1, 2017 and serves frail elders who are nursing-home eligible, allowing them to remain safely in their community. PACE is a model of care that provides an 11-member interdisciplinary team approach to create individualized care plans. PACE participants visit the center two, three, or five days each week to receive care. In-home care is provided to round out the TRU PACE spectrum of services.

PACE is a Medicare/Medicaid funded program, but start-up costs, along with costs to provide the PACE center are not reimbursed by these government insurance programs. Therefore, TRU Community Care has been raising funds through a capital campaign to help to offset costs associated with their construction of the PACE center. The Paige Foundation made a grant of $40,000 to assist with the capital campaign.

PACE requirements dictate that program staff must be in place before any seniors can join the program. When the Paige Foundation learned about the need to front the program costs, Schafer put the foundation and the organization together to discuss possible loan arrangements. The very favorable loan rate the foundation offered to TRU enabled TRU PACE to borrow $1.5 million.

TRU PACE Partners with Other Local Nonprofits to Serve Elderly in Boulder and Southwest Weld Counties

(Lafayette, CO) – TRU PACE (Program of All-inclusive Care for the Elderly), an innovative new program of TRU Community Care, is partnering with several area nonprofit agencies to serve the frail senior population in Boulder and southwest Weld Counties. Now open, TRU PACE helps seniors navigate the complex issues associated with aging.

The program provides a safety net for frail seniors ages 55+ who would otherwise need nursing-home-level care. Participants in TRU PACE arrive at the day center (thanks to transportation from Via) to receive interdisciplinary, coordinated care from an 11-member team, including a physician, nurse, nutritionist, physical therapist, and more. Services include recreational therapy, laundry, meals, and prescription management.

“PACE participants are what I consider the most vulnerable members of our community. Via, as the county’s nonprofit transportation provider, is happy to partner with TRU PACE to serve seniors in Boulder and southwest Weld counties.” said Mary Cobb, Director of Communications at Via.

Thanks to local partner organizations Via and Meals on Wheels – Boulder, TRU PACE participants receive mobility support as well as nutritious meals both on-site and to take home. TRU strives to keep these seniors independent and living in their communities for as long as possible.

Services include coordinated healthcare services and support; a day program, social activities, and physical therapy; and transportation to and from the TRU PACE facility provided by Via.

TRU Community Care Receives Worthy Cause Grant from Boulder County

(Lafayette, CO) – TRU Community Care, a nonprofit organization providing hospice and supportive services to the Boulder community and beyond, has received a $300,000 Worthy Cause grant from Boulder County.  TRU Community Care will allocate these funds (capital only) to its new TRU PACE facility located at 2593 Park Lane in Lafayette. TRU PACE stands for Program of All-inclusive Care for the Elderly.

Scheduled to open in early 2017, TRU PACE coordinates and provides all needed preventive, primary, acute, and long-term care services so that older individuals can continue living safely in their community. TRU PACE is an innovative model that enables individuals who are 55 years old or older and certified by the state to need nursing home care to live as independently as possible. TRU PACE will help participants and their family members navigate the complicated issues associated with aging.

In 2008, county voters approved “Worthy Cause III,” a 10-year extension of a .05 percent sales and use tax that helps with debt reduction (payments toward capital debts such as mortgages and loans for construction/renovation) and funding of capital facilities for nonprofit organizations. These funds help with the high price of obtaining and/or improving facilities that fit nonprofits’ specific needs as they serve the population. The tax expires in 2018.

Donations to TRU Community Care can be made here and support hospice care for uninsured patients. Occurring this year on November 29, #GivingTuesday is held annually on the Tuesday after Thanksgiving (in the U.S.) and the widely recognized shopping events Black Friday and Cyber Monday to kick-off the holiday giving season and inspire people to collaborate in improving their local communities and to give back in impactful ways to the charities and causes they support.

During 2015, TRU served 1,411 patients and families who were living with a terminal illness or with an advanced disease, offering their expert medical, emotional, and spiritual care and support.

  • 1,153 patients received hospice care
  • 258 received palliative home health and social work counseling
  • 422 stayed at the TRU Hospice Care Center at some point during their treatment

Of these patients, $67,495 was provided in indigent services to people who did not have the means to pay for care.

92Y − a cultural center in New York City that, since 1874, has been bringing people together around its core values of community service and giving back − conceptualized #GivingTuesday as a new way of linking individuals and causes to strengthen communities and encourage giving. In 2015, the fourth year of the movement, #GivingTuesday brought together over 45,000 partners in 71 countries and helped raise nearly $117 Million online in the US alone.
