Part-Time Care Center Social Worker

JOB SUMMARY
We have an immediate opening for a part-time (flexible shifts) social worker at our hospice inpatient unit (TRU Hospice Care Center) in Longmont. The social worker is responsible for providing psychosocial services for the team caseload. S/he provides direct services to patients and families, collaborates with the other Interdisciplinary Team (IDT) members within the care team and other healthcare providers in various care settings and may be asked to work in all areas of clinical services. SCHEDULE AND LOCATION
The Care Center Social Worker position is 20 hours per week (flexible days, flexible hours) and is primarily located at the TRU Hospice Care Center in Longmont, Colorado. Rotating on call.

EDUCATION AND QUALIFICATIONS

  • Graduate of an accredited Master’s Degree in Social Work program
  • A minimum of one year of experience in care setting that delivers end-of-life care
  • LSW preferred
  • Must successfully pass agency required background checks and drug screening
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100K Property Damage coverage) and have reliable transportation

ESSENTIAL DUTIES

  • Identifies the patient’s and family’s psychosocial, environmental, safety, and developmental needs and participates in individualizing the patient’s plan of care based on those needs
  • Incorporates patient/family preferences and goals within the plan of care
  • Supports and respects the patient’s and family’s unique spiritual and cultural beliefs
  • Facilitates family meetings
  • Identifies community resources for the patient and families
  • Meets the organizational expectations of a member of a care team
  • Participates in on-call system as requested; this includes evening, night, weekend, and holiday hours
  • Supervises students and interns as requested
  • Provides supervision and support for volunteers

SALARY
$28.08 – $42.11 per hour
